Active vs Inactive B Vitamins: Why the Form Matters

Walk into any pharmacy or health food shop and you'll find dozens of B vitamin supplements. But there's a crucial difference that most labels don't explain — a difference that determines whether a supplement is doing anything for you or simply passing through your system unused.
That difference is the form of the vitamin. And for the estimated 40–60% of people with MTHFR gene variants, it's not a minor detail.
The conversion problem
Most B vitamins sold as supplements are synthetic, inactive forms that your body must convert before it can use them. That conversion depends heavily on the MTHFR enzyme.
- Folic acid must be converted to methylfolate — MTHFR impairment blocks this final step.
- Cyanocobalamin must be converted to methylcobalamin, again requiring functional methylation.
- Pyridoxine must be converted to P-5-P in the liver.

The active forms your body can use directly
Methylated B vitamins are these same three nutrients supplied already converted — so no MTHFR step is required.
- Methylfolate (5-MTHF) — bypasses the MTHFR conversion block entirely and is immediately bioavailable.
- Methylcobalamin — the neurologically active form of B12, which participates directly in the methylation cycle.
- P-5-P — the active coenzyme form of B6, supporting the transsulfuration pathway for homocysteine metabolism.
What this means for choosing a supplement
If you carry MTHFR variants and you're taking a standard B-complex built on folic acid and cyanocobalamin, you may be getting far less benefit than the label suggests. Switching to active, methylated forms is the single most straightforward change most people with MTHFR can make.
One caveat worth knowing: methylated forms can feel noticeably stronger, and starting at a high dose is a common mistake. See start low, go slow before you begin.
